Once you have made your decision to Independently Home School your child(ren) you will need to provide the following:
- Register with the Home School Office by completing the Tennessee Intent To Home School Form
- Provide a copy of your child(ren) immunization on the Official Tennessee Certificate of Immunization T.C.A. 49-6-5001
- Provide proof of the required education (K-12 grades; high school diploma or GED)

Tennessee Notice of Intent to Home School
Bắn Cá Hải Tặc iOSThis Tennessee Notice of Intent to Home School may be utilized as notice of your intent to conduct a home school as permitted by T.C.A. 49-6-3050. This form is to be completed only by parent(s) or legal guardian(s) conducting a home school under the supervision of the superintendent of a local education agency (LEA).
A child must be enrolled each year the child is home schooled, the Notice of Intent to Home School form must be completed and submitted prior to the start of the school year. Should home schooling be discontinued after enrollment, Metro Nashville Public Schools must be notified.

Homeschool Attendance Form
The parent/legal guardian will need to keep a calendar that shows the child(ren) are receiving a minimum of 4 hours of instruction daily for 180 day school year T.C.A. 3050(b)(3). At the end of the school year the calendar is to be sent to the Home School office. Download the form here.
